Thank you so much for allowing us to help you with your Health or Medicare Options!  In order to make our time most productive for both of us, please complete the following information.  This will allow us to spend more time discussing options rather than just data entry.

 

Step 1- Complete the short form below.

Step 2- Pick a time on Laryn's schedule that works for you. After you complete the form we will have most all the information we will need to put options together for you.  When we meet, we only need you to bring Medicare Number.

Step 3- If we will be talking to both you and your spouse, you will each need to complete the form using separate email addresses for each of you.  You will only need to make one appointment with us as we will be able to meet with both of you at the same time.
For Medicare Related plans, if you are looking to make a change during open enrollment, make sure you schedule an appointment and we select a plan BETWEEN October 15 and December 7 for an effective date of January 1, 2024
For Health Insurance plans, make sure you schedule an appointment and confirm new plan selection BETWEEN November 1st and December 15th in order to change plans effective January 1, 2024.  If we meet between December 16th, 2023 and January 15, 2024, your new plan choice will be effective February 1, 2024.
